Repeat prescriptions

Clients: use the form below to request repeat prescriptions for your animal. Fields marked with an asterisk (*) are required fields. Please allow at least 2 working days notice so that your request can be authorised by a veterinary surgeon.

If you require medications for more than one pet, please submit a separate request for each animal.

We generally ask to re-examine your pet every 3 months, to look for any changes in the condition and for any side effects or new problems. The law requires that this check is done when prescribing prescription only medicines.
